Alistair , you & many other methodologists are having this people first approach. these are the same people who are responsible for cost & schedule over - runs , whether developer , owner , users , project managers etc. how people first approach will elviate all these evils ?

That's the wrong question. The question is, "what are the choices?"
The answers include:
(1) trust to a process, and the people mistakes will vanish
(2) put the people up front, and they get to make their own mistakes
Neither makes the mistakes vanish
