To me it looks as if all three are possible

From the spec:

The sub-element auth-method
configures the authentication mechanism for the Web application. The element
content must be either BASIC, DIGEST, FORM, CLIENT-CERT, or a vendor-specific
authentication scheme.

Yes, Everything in the web.xml including auth-method sub-element are case sensitive.
