generating pdf with writable fields

hi is there any one to help me to generate the pdf with writable fields like checkbox , textarea ....etc

user has to enter the details manually in pdf is ther any option xsl-fo

I don't think XSL-FO (and, accordingly, libraries that use it to generate PDFs, like Jakarta FOP) has a concept of forms. A library than can create PDF files containing forms is iText, but it doesn't have provisions for working with XML data. You'd have to write your routines for that.
 
With a little knowledge, a cast iron skillet is non-stick and lasts a lifetime.
