Just to save some time and head-scratching for those using Monson-Haefel's book for preparation: there is a minor erratum in the aforementioned book in p.48. Namely:

'minOccurs is always "0" and maxOccurs is always "1"'

should read

'minOccurs is either "0" or "1" (the default is "1") and maxOccurs is always "1"'
