What should be done on performanace tuning?
Changing source code? Changing design?

It would require any or both of the options you mentioned above. Depends on the situation

So tuning is a refine process. How does it fit in
RUP or XP?

Not necessarily, performance for some applications is taken is thought of in design itself. One cannot leave it for refinement phase if it is a time critical application.
