I start to read web service RHM book,
I see on page 49(and before too) Listing 3-12, that instance show a new namespace ---> "addr" but on page 48 Listing 3-11, schema declares a namespace with other name --> "mh" so why in Listing 3-12, instance of Document use another namespace name???

The answer to your question as per me is as follows

1. On page 48- the schema for address type elements has been defined and the target name space is the jwsbook.

2. listing of 3-12 shows how this can be used. they used xmlns prefix addr:....... That was just a e.g. how one can use address
you can use anything else.

3. the listing under 3.1.5 is not related to listing 3.12., this list under the section 3.1.5 uses mh as prefix, but itis the same namespace jwsbook. Also note that in 3.1.5 para author says this is part of listing 3.5 -

So not confuse the 2 listing on page 49.
