JSP and Forms

Hello- I am working on a project that was passed on to me. I am a junior developer and some help would be greatly appreciated. I have a jsp document that uses multiple page fragments to create dynamic content. One of those pages is supposed to generate additional form fields for users to input information. The user clicks on a plus button,which is an on-click event which inserts more fields. The minus button does the complete oppisite, by deleting the fields and the information inside. When I add new fields, the information is sent to SQL Server. When I delet a new field the information is deleted from SQL Server. The poblem comes when I have more than one field populated and I decide to delete one because the information is incorrect. I delete the field and then click the plus button for a new field and it duplicates the information from the first field and inputs it into the database. How do I keep it from duplicating? I have code samples, which I know will be helpful. Sorry for the detailed information, just thought it would help.
