You need to be much more specific about what the actual problem is you're facing. Subjects like Swing, JDBC and database modeling can't be taught in a forum like this.
Syskata, Sun has a sample J2EE app called "Pet Store." You can use this if you are just looking for an example that you can run out of the box.
