If I have a web application that uses
's, should I place the EJB in an EAR file and add the web application -- as a WAR -- to the EAR?

That's correct. You also need the XML file that describes the war and
so the appserver can find them when it wants them.
Customer surveys are for companies who didn't pay proper attention to begin with.

Your EAR File should look like this:
1a) .WAR File
1b) .JAR File
