When & by whom is this exception thrown??

The best answer I can give without more details is:
- The exception is thrown by the EJB Container
- The exception is thrown because someone called setRollbackOnly() during the transaction, some bean creation failed, or some other "fatal" error occurred that made the container mark the transaction as rollback only.
