That differs from provider to provider. Whichever one you choose will provide you with documentation that specifies what to do. Often it involves using FTP to put image or PDF files on the provider's server.

Googling for "efax" or "e-fax" will find many providers.

Some providers offer mail-to-fax capability, but not all do. Check the feature list before you sign up with one.

I did that once and I recommend

It has a mail-2-fax gateway and you can either send plaintext or PDFs. It's reliable and fast.
