JMS acknowledgement modes

I am trying to understand the JMS acknowledgment modes and I have three basic questions.

1.Who requests for an acknowledgement is it the consumer or the producer?
2.What does it mean that the client session has been acknowledged?
3.In DUPS_OK_ACKNOWLEDGE Mode, why is it that a message may be redelivered to the client again?

Many Thanks
