Welcome to Javaranch. I respect your eagerness to learn Swing however we do not just give people programs. This site is to help people learn Java.
I would recomend you start out with the Swing Tutorial from Sun. There are a lot of code samples and working examples throughout the entire tutorial.
Once you have grasped the concept of building GUI's in Swing you can move on to the JDBC Tutorial. And again, there are a lot of examples and code to help you out.
As you are working through the examples in Swing, you are welcome to ask questions here about what you are learning and especially when you don't quite understand something. Just post the part of the code you are having problems with and we are more than happy to give you a helping hand.
The same goes for JDBC. Just make sure you ask specific JDBC questions in the JDBC FForum here on Javaranch.
