tomcat can't find servlet

not sure if this belongs here or in the tomcat forum...
Tomcat 4.1.24 can't find my servlet and I don't know why...
My directory structure is:
tomcat/webapps/rl/signon for my jsp and html
for my servlet
My html has the following:
<form action="../servlet/signon.CreateAccountServlet" method="post">
but I get a 404 from tomcat when I click the submit. I've also tried replacing the .. with rl and /rl and it still gives me the same thing.
I also get the same error if I try to get to the servlet directly from my browser using:
any ideas?

Is the invoker servlet enabled (not commented out) in tomcat/conf/web.xml ?

do you have servlet entry in rl/WEB-INF/web.xml file ??
