Building JSP pages and link to MySQL table

Hi,I'm just starting on buiding JSP pages from scratch.
I need help to build my three JSP pages.Here is the requirements:

I have to design a JSP page (JSPFanClub.jsp) [page 1] that contains two buttons : "Add New" ; �Delete" ;
1 - Clicking "Add New " ,the JSP page (JSPMemberDetails.jsp) , [page 2] shows up .It includes fields FirstName, LastName, PhoneNumber,email with text Boxes each and Submit button.

When Submit is press, the message is sent to a table in MSQL. Also a Servlet need to be created ,this Servlet emphazise the use of INSERT and DELETE.

2 - Clicking on "Delete" bring me to (JSPMemberList.jsp) [page 3]. This page has one checkBox per record (Fistname,LatName,Email,PhoneNumber).For instance,if in my table I have
7 records , I would get 7 checkBoxes.If I select 2 records(checkmark) , those 2 records will be delete by clicking the Submit button and it will go back to [page 1 ] (JSPFanClub.jsp)

1-I want to know if I'm in the right path and also how to link [page2] to "Add New" button of [page1].

2- how to link the "Submit" button to the mySQL table Member

3 - how to make appear one checkBox per record (record is compose of
firstname,lastname,email,phonenumber)in the JSPMemberList.jsp [page 3]

4- from the JSPMemberDetails.jsp [page 2] how to design a servlet that use method INSERT and DELETE
