How does the exam question preparers come up with so many different questions for each exam

i think they might have a huge lot of questions and depending upon each exam, the questions are given, may be

my view - " they will be having a pool of questions for each topics - and a certain number of questions will be automatically taken from each pool.... "

Have you ever made the screening test for new hiring. Sometimes it goes like you are working on a real thing and encountered an interesting problem, and you make a question out of it.
