The black box says.."Assuming you have already configured username/password/role info into your server."

Where do we specify info in the server?

That's not standardized, and so it differs from server to server. For example, Tomcat has the concept of realms.

No. web.xml lists the roles needed for particular URLs, but it plays no part in configuring users, their roles and passwords.
Have you checked out Aspose?
