My First JSP Form with Struts

OK, I need some feedback, I feel like I didn't do this correct, but I am asking. It is a simple form one that I expect to look like this, but nicer than text

Here is my JSP page

Am I on the right track, or am I as off as I think I am.

Nope I am pretty sure now that I am way off. It looks nothing like the examples from the book I am reading.

What is it doing that it shouldn't be doing? Or what is it not doing that it should be doing?
