I have a help working, except for the index.
A real index, such as in a book, permits multiple entries, where appropriate,
to be made under each item. jhug.pdf documentation is silent on this subject.
Does anyone know whether there is a method, using the stated xml syntax,
to create multiple entries in the index, and if so, how?

The given syntax is:
<indexitem text="item-text" target-"map-reference">

Would something work like:
<indexitem text="item-text"
target-"map-reference1"
target-"map-reference2"
.
.
target-"map-reference-n">

or, some other construction
