Where Action class is located as per Struts architectural diagram ? Is it between Controller and Model?
When Action class is called? Whether during form submission?

The Action class is considered part of the controller.

The basic flow of a Struts app:

User makes a request -> ActionServlet -> RequestProcessor -> Action
