Here is a good link that tells you how to write a download action. There's also an example of this in the sruts-examples.war file that comes with the Struts download. Look at the org.apache.struts.webapp.exercise.ImageAction class.
Your question is rather vague...but I might be able to help. If you need to display the images inline on the page, then you need to set the URL of each image to that of an action that will read the image out of the database and return that image. I developed such code a few years ago. The requirements changed and the code was no longer needed so it has not been used in a production environment but I do remember it working. Here is a slightly modified version of the action code:

Ahhh...you got to love those types of requirements. In the client sever days the client always said "just make it work like Word/Excel" and now it is "just make it work like Amazon/eBay/Yahoo". I always wanted to grab the client, slap them and yell "that product took years to develop, cost millions of dollars and required teams of designers and developers...and you want me to do it in a week?!?!?"
