CMDEV: MySQL Developer Exam: A few questions

Hi,

I am planning to take the MySQL Developer Exam. Some questions I just won't get answered by google or the mysql website. Does anybody know?

* Current price for the exam seems to be 200$. But I don't find any information, whether this is 200$ for both parts or for each one of the two
* I bought the "Certification Study Guide" some time ago which is from 2005. I read that it contains a voucher for 25% off, but in the study guide itself I can't find the voucher. Is it on the CD or where can it be found? And will it still be valid? How can I find out?
