If the user checks "Send notify when a reply is post" in the new post form, an email alert must be send to all users which have this option checked for the desired topic ( or topics ).

I agree. Here's the link: http://ej-technologies/jprofiler - if it wasn't for jprofiler, we would need to run our stuff on 16 servers instead of 3.
