You become a Certified Scrum Master after taking a course. Unless you fail the online assessment, the cost of the exam is included with your course. See the Scrum Alliance FAQ

Just wanted to add some more details. There is no certifying body for Scrum Certification. There are a few websites like scrum-master.info which give you the different providers and also how and in what they vary. You can go through the same and pick the one that suffices your needs the best.

This is Jennifer Bellamy here. Recently I have taken up Scrum Master Certification. Based on my earlier research I can suggest few names.There are three hugely regarded certifications and they are from scrum.org, scrumalliance and scrumstudy.com and all are approved by all leading companies.
As mentioned by Matt Daniels, there is no certifying body for Scrum Certification.

Have taken up Scrum Master Training from SCRUMstudy (http://www.scrumstudy.com/) and I foun it fruitful. Like the teaching methodology and course content.

I would suggest you to visit:http://www.scrum-master.info/SCRUM-AGILE-Providers.asp. There are many Scrum Master courses available offered by training providers like Scrum Alliance, Scrum.org, SCRUMStudy.com etc. Pick the right one!

if any body in India is looking for Certified Scrum Master in India, they can visit at http://siantiindia.com/scrummaster.html. Me and many of my friends / colleagues did CSM from them. Very good faculty , and really enjoyable interactive session during the class. Mostly all of us got clear in exam in one shot. Fees is reasonable too I believe.
You can just catch them by sending a mail from their registration page , executive will cal you and will explain everything .
GET SCRUM CERTIFIED !!!
