Take a look at the GitHub repo for JD eSurvey https://github.com/JD-Software/JDeSurvey. It’s a survey web application built using the Spring.

I work at JD Software the company that published JD eSurvey. I am disclosing this in accordance with the Federal Trade Commission’s 16 CFR, Part 255.
