Hi Saleem Basha,
Please read this: EaseUp (<-- click)
It will help you understand why a subject like "I need answer asap" is counterproductive, especially for you, the poster.
The answer to your question: It's entirely up to you and your team what "document" this will be. Some teams have a spreadsheet or (shudder) Microsoft Project document or something similar. Others will just have sticky notes on a whiteboard to show requirements and tasks and their progress. How you access that information follows the form it's in, accordingly.

Whoa, EaseUp there partner!
I'm not quite sure I understand what sort of answer you're looking for. Are you looking for information on project management methods, like PRINCE2, or software development processes like Agile? Or both?
Edit: Ugh, not fast enough, as usual...
