How to receive certification sucess kit

hello guys i have sucessfully cleared my scja exam 1z0-850 with 96%.
I am stuck on one place , i dont know how to apply in order to receive the certification kit.I appeared for exam on 13th aug 2012 and today 28th aug 2012 i got time to apply for receiving the sucessful certification kit
Please guide me. I have already followed all the steps given in my temporary exam score report.
But i am not able to verify it in the certview.oracle.com i have only last attempt remaining now, their are total three attempts in which i have already used 2 which resulted in invalid please help me anyone ASAP.
