qn about project tracking & release notes tracking tool

I am looking for a opensource package that can be used for tracking project statuses & major changes to modules etc.

I am sure a project based wiki can do the trick, but I am not sure if there is other openssource tool that can be used for this purpose.

FYI I am not asking for a change management tool.
