How much do I need to know about design patterns for the test.
Is there a book that helps?

I found the J2EE design patterns online sufficient to pass part I and to complete part II assignment. The "gang of four" design patterns book is a highly recommended supplemental resource.
Infact, you probably won't need anything else other than these two.
