Special rankings assigned without setting it in the admin panel for user options

We're experiencing a mysterious problem with special rankings.

At one point all the users of a certain group were being given a special ranking, but it had not been defined in the Admin Panel. Then we "edited" that one special ranking without changing anything. (Just set something, set it back and saved.) After that, all new users were given a different special ranking.

Any idea what could be happening or what we need to do or correct?

Thanks.

PS It's a fresh install of 2.1.8

[edit]
I noticed that the Admin Panel has a cache reload option for Rankings, but it's not mentioned in the cache documention. What does it do?
Thanks again.
